Booking and Preparing For Your COVID-19 Test
Registration steps 1-4 must be completed in order prior to making your appointment. Click the link below to view each step or scroll down to view them all.
If you test positive for COVID-19:
Staff who have had a positive case should not be tested again for 90 days because they could still test positive long after they are symptomatic or infectious. However, it will be up to staff to mention their positive test to their Human Resources Director and Supervisor as the labs will not disclose the names of Staff members who have tested positive.
Step #1: Complete the Consent Form
Your consent is required to permit the Santa Cruz County Office of Education to share your identifying information and insurance plan information with Stanford Health Care. This form has 4 parts and you will receive an email confirmation upon completion.
You can find your insurance information on your insurance card.
Step #2: Review Email for Appointment
Within 24 hours after you complete the consent form your will receive an email from the sender: Santa Cruz COE COVID-19 STS with address firstname.lastname@example.org. Please check your spam for the email. In this email, you will receive your appointment based on your preference. There will be a link to reschedule or cancel your appointment if you wish. If you reschedule or cancel you will click on Forget Password on the Appointment Plus Website. You will then type in your work email address, will receive a email to reset password and will then ask to reset your password one more time.
Within 5-10 days of completing the consent form you will receive an SMS text message from Stanford MyHealth to setup your account - please complete within 2-hours of receiving these instructions. Contact the MyHealth Service Desk at (866) 367-0758 if you are not able to activate. You will not receive test results if you do not activate your account via the SMS instructions. Please call MyHealth All test results will be available in your MyHealth Account within 24-72 hours of your test completion. You will receive an email when they are available.
Do not attempt to schedule a COVID-19 test through the MyHealth site.
Valencia Labs (Color) Tests
Your email confirmation will confirm if you are signed up for Valencia Labs (Color) tests. For your first appointment with Valencia Labs you will be required to complete 5-10 minute state registration for this system. Your confirmation email will direct you to this link, but you can do this at any time on the Color Registration Website for Santa Cruz COE. If you do not complete prior to arriving, you will be asked to complete registration onsite.
Step #3: Review How-To Administer Nasal Swabs
This COVID-19 test is a self-administered nasal swab collection.

It is required that you watch the following two-minute how-to video and review the following diagram prior to your appointment to prepare yourself for how to collect a sufficient specimen.

Please review the documentation below prior to arriving at your appointment.
Step #4: Wait for Email Confirming your assigned appointment - 2 Days Prior to Appointment
All testing will be conducted by appointment only. We cannot accept walk-in appointments. The first time you log in to the Appointment System, you will need to click "Forgot Password" and enter your email address. You will receive an email from Appointment Plus with a link to set a new password. Click on the link in your email to set a new password for the Appointment Plus. Once logged in you will be able to change your appointment and add another if you would like.
We ask school staff not to report to the testing site if they are experiencing any COVID-19 Symptoms: Fever, chills, Cough, Shortness of breath, difficulty breathing, Fatigue, Muscle or body aches, Headache, New loss of taste or smell, Sore throat, Congestion or runny nose, Nausea, vomiting, or Diarrhea. We ask school staff not to report to the testing site if they have been exposed to COVID-19. In addition, all individuals will be screened for COVID-19 symptoms upon arrival to the test location and will be required to comply with all COVID-19 safety precautions including physical distancing and the use of face coverings while at the testing location.

Step #5: Prepare for Day of Testing
Arrive at your testing location 1-2 minutes early so you can be on-time to the first station. The following flowchart will explain the location of :
Screening Station - Temperature and Symptom Check
Check-in Station - Confirm identify, collect testing kit (Stanford), setup recurring appointment (optional), follow-up the signs for which lab you will be testing with
Testing Station - Administer self-swab test with trained with staff
Step #6: Where to View Your Test Result
You will receive an email or text message informing you that your COVID-19 test result is available to view in the Stanford MyHealth portal OR the Color Website that is referenced in Step #2. Test results will be posted within 24-72 hours of your test. For Valencia Labs (Color) you may need the barcode of your testing vial. Please take a picture or retain the hardcopy provided during testing.
